Legal guardian photo ID

  • Valid Massachusetts driver’s license
  • Valid Massachusetts ID card
  • Passport
  • Military, school, or employment ID

Student birth certificate

  • Or proof of legal guardianship from a court
  • Or proof of temporary custody from DCF
  • Caregiver affidavit not accepted

One proof that you own or rent at a Boston address

  • Deed (Go to masslandrecords.com/suffolk and type in your address to download yours)
  • Most recent mortgage statement (dated within the past 60 days) or a property tax bill (dated with the last year)
  • Current lease (including BHA and HUD leases)
  • Section 8 agreement
  • Executed public housing recertification
  • Notarized Residency Affidavit (dated within the past 60 days)

AND

Two proofs that you live at that Boston address (dated within the past 60 days)

  • Utility bill (Gas, electric, or oil bill; water/sewer bill not accepted)
  • Home telephone bill (Landline only; cell phone bill not accepted)
  • Internet, satellite, or cable bill
  • Most Recent W-2
  • Bank or credit card statement
  • Car registration, vehicle excise tax bill, or car insurance bill
  • Letter from an approved government agency (Departments of Revenue [DOR], Children and Family Services [DCF], Transitional Assistance [DTA], Youth Services [DYS], Social Security, any communications on Commonwealth of Massachusetts letterhead)
  • Letter from shelter (if homeless)
